A pioneer and leader in the orthodontic profession, Dr. Damon is widely known for development of the Damon System and its philosophy of use and has lectured worldwide on advanced treatment protocols. He earned his DDS and MSD from the University of Washington School of Dentistry and was awarded its Distinguished Alumnus Award, one of only 23 alumni to have received it. In private practice for more than 40 years, Dr. Damon has written numerous professional articles and has been featured in leading peer-reviewed journals, including the JCO and the AJO-DO. A chapter he wrote was accepted for inclusion in the text Orthodontics: Current Principles and Techniques, edited by T.M. Graber.

Dr. Kozlowski received his DDS and orthodontic specialty certificate from State University of New York at Buffalo. In private practice, he has lectured all over the world, including for the AAO and its constituent societies, for universities, study clubs, the European Damon Symposium and annually for The Forum. His topics include digital orthodontics, efficient early treatment and facial esthetics. He has been published in several orthodontic journals, including Seminars in Orthodontics and the Journal of Clinical Orthodontics. A fitness advocate, he has completed five Ironman Triathlons, 12 Half-Ironman Triathlons and numerous marathons and endurance cycling events, including the grueling Mt. Washington Bicycle Hill Climb seven times.

A diplomate of the American Board of Orthodontics, Dr. Chang received his PhD in bone physiology and certificate in orthodontics from Indiana University. He is publisher of the International Journal of Orthodontics and Implantology (iJoI), author of the workbook for the International Association of Orthodontists and Implantologists and co-author of Orthodontics, Volumes I, II & III. In private practice, Dr. Chang also lectures worldwide on a wide range of topics, including impaction therapy, PSL mechanics, gummy smiles, TADs, and Steve Jobs’s effective presentations. As a private instructor, he has taught 2,000+ doctors from 21 countries and is actively involved in the design of TADs and their application in impaction treatment.

Dr. Frost received his dental degree from the University of the Pacific School of Dentistry and practiced general dentistry for 5 years—two with his father and three in his own office. Following a one-year fellowship in temporomandibular joint dysfunction, he completed a two-year residency in orthodontics and dentofacial orthopedics at the University of Rochester. In private practice, he is currently a part-time associate clinical professor at the University of the Pacific orthodontic program and has lectured on the Damon system globally. Dr. Frost has been named one of the Phoenix area’s top orthodontists by a group of his peers from 2004-2015.

Mike Mayhew, DDS, MS, received his dental education at the University of North Carolina with specialty degrees in pediatric dentistry and orthodontics. He is board certified in both specialties and operates a dual-specialty practice in Boone and North Wilkesboro, North Carolina. Dr. Mayhew has lectured nationally and internationally on the Damon™ System, CAD-CAM digital orthodontics, indirect bonding, and office utilization of CBCT. He is on the Sports Medicine Team at Appalachian State University, is an adjunctive clinical professor at the UNC School of Dentistry, has held leadership positions in organized dentistry, and serves as the director of the North Carolina/South Carolina Damon Study Club. He was inducted into the American College of Dentists in 2010 and the International College of Dentists in 2013.

Screening, Planning and Treatment

Dr. Elizabeth Menzel | Herrsching, Germany

Orthodontics is increasingly confronted with ancillary health issues such as TMJ disease. For most orthodontists, TMJ disorders seem like insurmountable hurdles. In her presentation, Dr. Menzel will demonstrate simple techniques to screen patients during initial consultations—either by the doctor or a staff member—to evaluate whether there is definitive evidence of TMJ disease present or whether there are underlying diagnostic indicators that can lead you to a treatment alternative which can help the patient circumvent the need for more aggressive treatment in the future. She will also offer treatment protocols different from conventional methods, briefly demonstrating such protocols with various cases.


Dr. Menzel earned her degree in dentistry from the University of Stellenbosch (South Africa), did postgraduate training as an orthodontist in the practice of Dr. Brigitte König-Toll (Kronberg, Germany) and in the department of orthodontics, University of Kiel (Germany), undergoing her examination for specialization in orthodontics from Dr. Wolfgang Schopf, University of Frankfurt (Germany). In private practice, she has lectured throughout the world with her primary focus today on interdisciplinary biodynamic orthodontics and orthodontic treatment of the temporomandibular joint from diagnosis to pathology-free centric relation treatment results.
